Violence (non-sexual) Risk Assessment and Report (flat fee): $1,800

Fitness for Duty Evaluation and Report (flat fee, plus additional fees): $1,250

Workers Compensation/Disability Evaluations and Report (flat fee, plus additional fees): $1,250

Child Custody Evaluation and Report – Retainer: $4,000

Differential Diagnostic Evaluation and Report: $1,500

Cognitive Testing and Report: $1,500

General Psychological Evaluation and Report – Hourly Fee: $200/hr

Additional Fees

Records Review (>100 pages): $175/hr, pro-rated

Consultation with collateral sources: $225/hr, pro-rated

Travel time in excess of 1 hour, round trip: $250/hr 

Travel time less than 1 hour, round trip: $150

Individual Therapy/Treatment:

Individual Sessions with Dr. Bono: $200/hour

Individual Sessions with Madison Nohren, LMSW: $140/hr

Group Therapy/Treatment

75 Minute Therapy/Treatment Group: $300/month, $80/week

60 Minute Support Group: $50/week
